The AltaPoint Main
Menu. This is the initial program menu and the doorway to
all forms and operations in AltaPoint. |

User Information
Form. Enter your practice name, program serial number and
activation key. You can also specify your default provider code,
location code, as well as other options. |

Patient List.
Patient lists, procedure code lists, and diagnosis code lists can
be sorted by either name or code. You can quickly locate any record
in the system by entering the appropriate information in the FIND field
and selecting the GO button. |

Patient Form -- Page 1.
Use this form to view or edit the patient's name and address
information. |

Patient Form -- Page 2.
Selected from the patient form -- page 1. Use this form to
view or edit the basic insurance information for the patient. You
can also record the patient's referring physician as well as other options
and flags. |

Procedure Code Form.
This form is used to enter and maintain all of your procedure
(CPT) codes. These codes can then be easily selected from the
patient encounter form. |

Diagnosis Code
Form. This form is used to enter and maintain all of you
diagnosis (ICD9) codes. These codes can then be easily selected from
the patient encounter form. |

Patient Encounter Form.
Use this form to record patient encounters and enter appropriate
diagnosis, procedures, and billing detail. |

Encounter Diagnosis
Form. Selected from the patient encounter form. Use
this form to enter the diagnosis for a patient encounter. |

Encounter Procedure
Form. Selected from the patient encounter form. You
can enter up to 12 procedures per encounter. You can also use this
form to record patient payments. |

Select Diagnosis Form.
Selected from the encounter procedure form. Once you have
entered the diagnosis for a patient (see Encounter Diagnosis Form above),
you can specify the individual diagnosis that corresponds to each
procedure performed. |

Patient Notes Form.
Selected from the patient encounter form. Use this form to
record detailed notes and other narrative
data. |
